I often start from determining my student's level as well as weaknesses and strengths. After that, I can build my lesson around that. Often times, I focus on rebuilding the student's foundation knowledge of a subject, making sure that he or she understands thoroughly a subject and how can we arrive at answers consistently. I teach my students knowledge and how to convert it to good test scores, not just focusing on quickly improving scores and gaining little knowledge. In a lesson specifically, I often follow textbook lesson, starting with theoretical knowledge and let the student make examples to test their understanding, and then apply it to exercises. Periodically, I organise exams to let the student see his/her progress.
